Letter of Completion of Undergraduate Thesis Examination and Identity of Students who are Taking the Undergraduate Examination

Students who will take the Undergraduate  Thesis Examination are expected to fill out and submit the attached form as a requirement for taking the examination. The attached document consists of:

1. Thesis Examination Completion Certificate

2. Identity of Students Taking the Bachelor's Examination
